Adults Self-Assessment Rating Scale (ASRS)

This is a screening measure for ADHD. There are 18 questions. Try not to overthink the answers, but choose the answer that first jumps out at you. 

Please note that no questionnaire can make an accurate diagnosis of ADHD symptoms without the clinical evaluation of a professional. The results of this questionnaire is simply a guide, and is used to indicate whether it would be worth pursuing an assessment of ADHD.
