This is a screening measure for ADHD and Autism in adults based on the 

Adults Self-Assessment Rating Scale (ASRS) and the AQ 10

Please note that no questionnaire can make an accurate diagnosis of ADHD or Autism without the clinical evaluation of a professional. The results of this questionnaire is simply a guide, and is used to indicate whether it would be worth pursuing an assessment of ADHD, Autism, or both.
